65 y/o Male Parient - Cellulitis

A 65 year old male patient, resident of Nalgonda, weaver by occupation came to the OPD with 


Chief Complaints: 

Swelling of right leg since 5 days 

Fever since 5 days 


History of Presenting Illness: 

He was apparently asymptomatic 5 days ago, then he developed increased swelling in right lower limb, sudden in onset, gradually progressive, (foot to thigh), associated with pricking type of pain, aggravated on walking and relieved on rest. 

Associated with blisters on leg, local rise of temperature, fever, skin changes and pigmentation.


Fever, moderate grade fever, sudden onset, progressive, not associated with chills and rigour. 


Past History: 

Known case of filariasis since 15 years 

On medication intermittently when he notices increase in swelling of limb (Diethylcarbamazine) (Chymerol Forte)

N/K/C/O Hypertension, Diabetes, Tb, Epilepsy, Thyroid 


Personal History: 

Diet- Mixed

Appetite- Decreased 

Sleep- Adequate 

Bowel and bladder: Regular 

Addictions: Consumes alcohol consumption 

No Allergies 


Family History: 

Insignificant 



General Examination: 

Conscious, Co-operative, Coherent 

Temperature: 98.6* 

PR: 72 bpm 

RR: 21 cycles per minute 

BP: 100/60 mmhg


Pallor: absent 

Icterus: absent 

Cyanosis: absent 

Lymphadenopathy: present 

Edema: present


Systemic Examination: 


Respiratory: 

Vesicular breath sounds heard 


CVS: 

S1 S2 heard 

No murmurs heard 


CNS: 

No focal neurological deficit


Abdomen: 

Soft and non tender



Treatment history:

 1)IV fluids (normal saline 75ml/hr)


2) Inj.PAN 40 mg OD


3)Tab.Dolo 650 mg 


4) Inj. NEOLOL 1gm

Causes and treatment of Tachypnea?  Tachypnea is a condition that refers to rapid breathing. The normal breathing rate for an average adult is 12 to 20 breaths per minute . Tachypnea does not necessarily have a pathological cause. For example, exercise can cause tachypnea. Some pathological causes of tachypnea are sepsis, diabetic ketoacidosis, respiratory issues such as pneumonia, carbon monoxide poisoning, pulmonary embolism, pleural effusion, asthma, or chronic obstructive pulmonary disease (COPD). Other medical issues such as allergic reactions, anxiety
